He was first commissioned on 30th November 1892 and became a Major 30th November 1910 in the Indian Army. He died on Tuesday 18th April 1916 (Aged 44 on C.W.G.C. site)*(Spelt Koiekenbeek on C.W.G.C. site)

Location of Memorial:
Basra memorial, Iraq, Panel 53 and A memorial tablet fixed beneath a stained glass window in the south aisle of St Augustine's Church, Grove Park.

128th Pioneers Indian Army. Aged 45.

This Tablet Is Erected By His Wife 1917.

Note:
Memorial was in Basra war cemetery. In 1997 moved by presidential decree by 32 kilometers on road to Nasiriyah This was a battleground in the first gulf war, memorial is to 40,500 members who died in operations in Mesopotamia autumn 1914 to end of August 1921.
